How to download the above forms and use:

To save the PDF file directly to your PC, move your mouse over the picture of file name above, right click mouse button and select "Save Target as". You will be prompted as to where you want the file saved on your PC. After which, print out the form, fill up the particulars and mail it back to us with your payment.

Membership Payment Mode:

We currently only accept Company Cheque. Payment to be mailed to our Day Spa Association-Asian Chapter office in Hong Kong. Further details are available in the application form.
